- This is my first loan with NBCU.
- I have not had a loan with NBCU in the past 3 years.
- I am applying for a real estate secured loan.
- I was directed by NBCU staff to complete the full application.

Completion of the Advance Request is required if you can answer YES to any of the following questions:
- I currently have a loan in repayment with NBCU that is not secured by real estate or is not a credit card account.
- I have had a loan, which was not secured by real estate or a credit card account, with NBCU in the past 3 years.
- I was directed by NBCU staff to complete the Advance Request.
